Introducing Barton Creek Assisted Living

Barton Creek Assisted Living has served as Davis County’s premiere assisted living residence since 1999. We specialize in providing independent living with supportive assistance and around-the-clock nursing care. Services include meal preparation, medication management, incontinence assistance and much more. With 62 apartments we are large enough to provide a variety of activities including religious services yet small enough to provide intimate, personalized care for each of our residents.

Apartments are available in three basic floor plans; studios, suites, and one-bedrooms. Prices vary according to location, size and view. Contact us for exact pricing.

One-bedroom Apartments Basic room rates include all of the following services:

• Three delicious, home cooked meals served restaurant style per day.
• Daily bed making and trash removal.
• Weekly housekeeping, including linens service.
• Comcast Cable channels 2 through 99.
• Transportation services within the Bountiful area.
• Social, recreational, educational and religious activities.
• 24 hour emergency call response, plus on-call assistance.
• Regular nursing assessments and medication reviews.

Those requiring additional assistance on a recurring basis will be placed on a care level according to the extent of their needs. These care levels and their associated costs are outlined below:

Level 1
• Regular assistance with one activity of daily living.
• Medication Assistance.

Level 2
• Regular assistance with two activities of daily living.
• Medication Assistance.
• Weekly Personal Laundry.

Level 3
• Regular Assistance with three activities of daily living.
• Medication Assistance.
• Weekly Personal Laundry.
• 2 Hour Checks.

Level 4
• Regular Assistance with four activities of daily living.
• Medication Assistance.
• Weekly Personal Laundry.
• 1 Hour Checks.

Additional Costs
• A $500 deposit entitles residents to be placed on a waiting list to be called regarding available units. This $500 deposit will be completely refunded at any time if you decide not to pursue residency. When you move in to Barton Creek this deposit will be applied to the $1,000 move-in fee and becomes non-refundable.
• A second person fee of $700 / month is charged to couples
• Comcast cable basic programming is provided by the facility. Additional programming is available but will be billed to the resident by Comcast.

Location
Barton Creek Assisted Living is located between the South Davis Community Hospital campus and Lakeview Hospital at 499 East 500 South, Bountiful, UT. Getting here: travel East on 500 South past Orchard Drive (400 East). Turn left at 500 East. Barton Creek is the first driveway on the left.
